Mechanical ventilation is required when other modes of oxygen delivery fails. Sometimes ,there is catastrophic presentation of patient giving no time to use other mode of oxygen therapy and directly a patient has to be put on mechanical ventilation.

The main indication of mechanical ventilation is respiratory failure.Respiratory failure may be due to

causes affecting respiratory centre in the brain, causes affecting spinal cord damaging anterior horn cells,causes affecting neuromuscular junction and causes affecting muscles involved in respiration

On ABG report, if PaO2 is less than 60mmHg or Pco2 is more than 60 mmHg when the patient is breathing at Fio2 of 60 %, mechanical ventilation should be started as this is a state of respiratory failure
